Here are my configuration:
AppleTV 4k (tvOS 13.2) --> HDMI --> LG OLED65C9 --> eARC HDMI --> Denon X-4500H
MrMC v3.9-2
I used Amazon FireTV with Kodi in the past without problems. Now I installed my new AppleTV and MrMC. I testet it with 4 or 5 movies and all have the same problem. The sound from the center come from the speaker rear left.
I've tested it with this video, among others: Ant-Man.2015.German.DTS.DL.1080p.BluRay.x264-EXQUiSiTE.mkv
Movies from iTunes works fine. I also tested the same Ant-Man video with Plex and I had no problems.
I uploaded the debug log (8V91H.log)

It should be devices>receiver>TV when you have a nice receiver. The only reason to use ARC or eARC is when you're watching apps directly on the TV or if you only have a soundbar that doesn't support HDMI passthrough.

The problem I see is that you've spent a large sum on a nice receiver only to play stereo through it. TVs don't passthrough PCM well. I can't tell you why Plex doesn't have the problem. What are your audio settings in MrMC?
Anyway, if you turn on CEC for all devices just clicking a button on the apple tv remote should wake up the receiver and the TV if you go apple tv > receiver > TV. Then you get nice things like Atmos

Also, even with my receiver turned off I still get passthrough to the TV. So, with the receiver turned off my TV will then play the audio.
So, even if you didn't want to use CEC for one touch control using the siri remote you still have no reason not to use things properly.

I am getting the same issue
ATV 4k- LG WebOS - Marantz eARC
My son also has the same issue from his ATV 4k on multiple setups. He now uses infuse.
Setup is the same as anadkor above but different Rx Marantz SR5007
In any case my son has tried various configurations to no avail. Clearly the old bug has resurfaced!

I tested a while with different settings, apps and movies.
I have perfect sound with Dolby Atmos and everything fine with any iTunes movie. With the same Ant-Man MKV everything is ok with Plex and Infuse.
I also tested this movie with my old Amazon Fire TV with my old Kodi installation and it worked fine.
My Denon receiver shows 5.1 input channels and also Dolby Atmos (if connected to the Apple TV and Atmos is available for a movie).
I tested also with Dolby Testsounds and I can confirm that only MrMc has a problem that the center sound come from rear left.
And I don‘t want to discuss if it is a good idea to connect the ATV to the TV and the TV to the receiver or not. Only MrMc has a problem with this setup.
If my NAS would be fast enough, I would use Plex instead of MrMc.
