A library scan on start-up only happens when I've force quit MrMC (double tap Home button to get the app switching mode and swipe up).
Perhaps a Shutdown button in the bottom of the screen next to Favorites? Just as Kodi has?

We need to be able to load a file as soon as its on the pc nas ect. Cant it just autoupdate like infuse?

What do you propose? Force kill to get "library scan on start-up" to work seems unsatisfactory for a dev
